An unanswered question can disappear from a closure plan when someone removes an untidy note or marks a task "sent to adviser". Keep the question open until the relevant answer has actually been obtained and understood.

The register should explain what is unknown and why it matters. A list of vague concerns gives the next person little basis for action.

Make the question answerable

Write a specific question, the evidence currently available, the missing fact or advice and the decision affected. Name the person responsible for obtaining the answer and the expected next update if known.

Compare "sort out old machine" with "establish who owns asset A7 before including it in a marketing instruction". The second identifies both the question and its consequence.

The authority map helps identify which decisions need a confirmed route. The working records index can point to the evidence already found, so the next person does not repeat the same search.

Distinguish progress from resolution

A question can move through practical states: information requested, evidence received, professional review underway and answer confirmed for the stated purpose. Use whatever labels suit the business, provided they do not confuse an email sent with a matter resolved.

A fictional coordinator receives an invoice relating to A7. That is useful evidence, but it may not settle a disputed ownership history. The register should record receipt and the next review step rather than close the question automatically.

If an answer is conditional, retain the condition. If it addresses only part of the question, leave the remaining part visible and assign the follow-up.

Hand over the next action and its limit

When responsibility changes, ask the recipient to explain what they will do next. They should also know what action remains dependent on the answer and whom to consult if the situation becomes urgent.

The final records handover should include the live register, not only completed decisions. Keep private or professionally sensitive information in the appropriate controlled record and use a reference where sufficient.
